What are the most important concepts in eyelid surgery?

Without question, the most important concept is being conservative with skin excision while making a major effort to preserve, or even add to soft tissue volume. Excessive removal of skin or fat from the upper or lower lid will produce a hollow appearance, which actually ages the face. The goal of cosmetic eyelid surgery is to produce a rejuvenated appearance and in the eye area a youthful appearance is an appearance of fullness. With respect to the upper lid, skin excision should always be conservative and in some patients a conservative browlift will actually elevate and eliminate the need for skin removal from the upper lid. I almost always add some soft tissue volume to the upper lid, brow junction in the form of fat grafting and also commonly add fat to the tear trough and lower lid, check junction. With respect to the lower lid, some lower lid issues can be treated without an external or skin excision, while some patients require the external excision. Whatever the case, the approach should be the most conservative procedure that produces the intended outcome. In many patients with lower lid surgery, suspension and tightening of the orbicularis muscle is required and many patients also require some form of canthal support to support the lower lid postoperatively. Make sure that the surgeon you see can address all of these issues: a conservative approach to skin excision, a maintenance of fat – and ideally an addition of fat in some cases, and a conservative approach to the lower lid that includes some form of lower lid support postoperatively. If your surgeon addresses all of these issues you are most likely to have a beautiful and natural appearing eye area rejuvenation.
